I have this problem with many games. These black squares appear in some games for a very little time. I want to know what's causing them? and how to fix it. is it a faulty graphics card?

It happens a lot in Battlefield 2142, Battlefield 2. But some other game that i have had for a long time, like COD4, has no problems.
Almost all of the games that i play crash sometimes and then recover again.
Sometimes it ends up with BSOD. For Example, Crysis, Flight Simulator X, Prototype, CSS, World Of Warcraft work fine.. no crashes or anything but Fuel - Crash/BSOD, Cod4 - Crash, Call Of Yuarez: Bound In Blood - Crash/BSoD, Riddick: Assault On Dark Athena - Constant Crashes and BSOD (UNPLAYABLE) and that list can go on forever i guess.
It's one of those black screen -> ''Display driver atikmdag stopped responding and has successfully recovered'' (-> Game crash) Errors. Some games work fine, but some start messing too often so it has become unplayable. What can i do to fix this problem or at least find out what's wrong?

3GB RAM (1x 2GB, 1x 1GB)
Club3d HD4850 512mb Overclocked Edition (Latest drivers)
Xilence 580W PSU - Model: XP580 or XP580.(12)R
Windows Vista Home Basic 32-bit Service Pack 2
RAM - tested, No errors with over 700% coverage
Scanned whole PC With AVG Free - Clean
Scanned with spybot S&d - Clean
NO overheating problems.
Oh and the PC is 2-3 years old. I have upgraded RAM (originally 2x1GB, PSU (Originally a crappy 400w PSU) and GPU (Originally POV 8600GT 512mb). The warranty of my graphics card is still active...
I have NOT overclocked my PC in any way.
My ram... i switched back to 2x1GB sticks that originally came with my pc.... the problem is the same.
For the first time EVER, I got something like this after playing Resident Evil 5 for a very short time.

It only happens in THAT game. When this happens, i can still hear the sounds, the cursor works fine.
