This is the only thing I see there:

mchInjDrv: \??\C:\DOCUME~1\User\LOCALS~1\Temp\mc21.tmp (disabled)

Click on My Computer. Go to Tools > Folder Options. Click on the View tab and make sure that "Show hidden files and folders" is checked. Also uncheck "Hide protected operating system files" and "Hide extensions for known file types" . Now click "Apply to all folders"
Click "Apply" then "OK"

Go to the forum here and upload the C:\Documents and Settings\User\Local Settings\Temp\mc21.tmp file.

Here are the directions for uploading the file:

Just click "New Topic", fill in the needed details and post a link to your thread here. Click the "Browse" button. Navigate to the file on your computer. When the file is listed in the window click "Post" to upload the file.

Apparently it is a hidden file and very well be the problem one since its hidden. So how can I expose this file?
Post another Hijack This log and another startuplist please.

Hi.............not sure whether XoftSpy has anything to do with it but lets kill its scheduled job.

#######################################

Disable ALL spyware scanners please.

Download: Microsoft Task Scheduler Command Line Utilityfrom here:
ftp://ftp.microsoft.com/reskit/win2000/jt.zip
or here:
http://mvps.org/winhelp2002/jt.zip

Unzip and copy jt.exe to your Windows folder. C:\Windows
or C:\WINNT

Copy/paste the BOLD below to Notepad and "Save As" KillJobs.bat
In the "Save as type" select: All Files

@echo off
jt /sd XoftSpy.job

Copy KillJobs.bat to your Windows folder.C:\Windows
or C:\WINNT
Double-click on KillJobs.bat
(when prompted, allow the file to run)

Delete KillJobs.bat

Post a new HijackThis log after.
