Post Feedback Form, need opinion & suggestions
Please have a look at the attached picture of a feedback form. It is to be kept on a web-site. Please let me know how it is. Also, please give me suggestions to improve the layout and appearance.

Secondly, there is one blank patch below the Submit and Reset buttons. There is no code, no tags because of which it comes. I am not able to remove that blank space.

It appears in Internet Explorer 6 but doesn't appear in Netscape 7. (But there are some other problems with Netscape 7 . When will we have perfect standards ).

I think the form is clean and has a good structure. Very well done. My question would be why you want all the information you request a user to complete?

The space between the buttons looks natural and as it should to me. Are you just not happy with the way it looks?

Thanks for the compliment . I am somewhat happy. But I know there is always some scope for improvement. So I am just trying for that and inviting suggestions

I will not make any part of information compulsory. So it a visitor wishes to skip a part, its upto him. But I should try to gather as much as I can.
