I get the following output from fdisk and cfdisk on my computer.

My list of drives in Windows shows:

Master(C:)
(D:)
Drv3_vol1(E:)

When I try to access drive D: from 'My Computer' in Windows I get:

Disk D is not available
A device attached to the system is not working.

My questions are:

Why can't I access drive D: from Windows?
Why doesn't cylinder one(1) show up in one of the outputs?
Can the situation be fixed or must I do a complete reformat?

Windows doesn't support Linux filesystems.
You could try explore2fs or ext2fsd
