When did the problem begin?
Can you associate the onset of the problem to the addition or removal of hardware or software, to include an update to Windows or your antivirus program? Have you run a registry cleaner recently? I am NOT suggesting that you run a reg cleaner.

"my icons keep changing to other things"
Do you mean they change from "extra large" to something [ large, medium, small, tiles, details ], or do you mean that the icons actually change design?

Did the problems you describe begin at the same time?

Have you tried a system restore?

Which scans for viruses and spyware have you done?

What does this mean?
".a whole load of viruses XD..."
If you mean that your computer has a lot of viruses, that could explain any or all of the problems you note.

Was this a clean install, or did you upgrade from something?

512 MB ram may be enough for you, but it is not sufficient for Vista. Insufficient memory could cause the desktop refreshing problem and the "programes and features it wont refresh and let me uninstall something else" problem.
