Are you removing a USB drive or thumb drive with out using the "Safe to remove icon" or the right clicking of the device in Windows explorer and selecting "Eject"?

By not removing USB drives in the proper manner, WILL leave tracks, and the system will get lost and produce some of the errors when the device is not found.

If you want to remove a USB drive without first flushing the cache, go to Device Manager and be sure it is optimized for "Quick Removal".
