How good is a virus program on small files that download?

Many (most?) programs now will give you a small EXE file.
When you execute, the exe downloads the actual program.

So... virus checking the exe will most likely show nothing at all.
But the exe file, once executed could downloads loads of harmful unwanted things?

The virus monitoring program may well still stay running...
But does nothing and can't detect a virus has just been installed?

Just wondering.

Something to consider:
I no longer use MS Windows, but when I did use Win 7, I had installed Sandboxie. Downloads first went into the sandbox, then checked for malware and virus before transferring to my Documents folder.
I still used quality anti virus and anti malware apps.

There's no 100% safety net, but that worked well for me.

AV doesn't just scan the first thing you download and ignore anything it does after. AV will also scan any files that are downloaded or executed by these smaller downloaders and it also scans programs and running programs on your computer continuously.
