Acrobat 5.0: Quark Source Table of Contents Doesn't Work

Does anyone have experience converting a document that has a table of contents into PDF format using 5.0?

When I try using several different docs, it does not recognize that there is a table of contents and therefore does not create the necessary "bookmarks" for the TOC. Please advise, this is urgent!

That depends on the source doc, I think. What is it?

The original doc in in Quark and has a table of contents. It will convert to PDF just fiine, but will not turn the TOC into a bookmarked list.
