Check box in Word for Office 97 SR2

    I am trying to create a template for my department through a word doc for them to fill in, I know there's a feature in Office that allows us to create a check box and let us tick/ untick on it.

    i try going to format and bullets numbers but to no avail. Any idea of how to do it. Appreciate your help.

    Save the document as a template (.dot) as opposed to a regular document (.doc).
    Go to File, Save As, and in the Save As Type box, select "Document Template".

    Then, right click in the toolbar area and open the "Forms" toolbar. One of the options on it is the Checkbox. As with other form fields, you can double click it to set the form control options.

    Just to add...

    When you've done adding your form fields, click the "Protect Form" button (with the lock on it) and then the user can 'check' the checkboxes etc.
