Conditional formatting colors randomly change

I have an Excel sheet with conditional formatting. The values in the cells are coming from a lookup table, and it's pulling across the values just fine. I've color coded the numbers using conditional formatting into 3 brackets, according to what the numbers are. Each time I save and reopen the document though, the colors have randomly changed to different ones! Any idea why? Here are the conditions I have set:

Cell Value is between 0 and 99 = green
Cell Value is between 100 and 499 = yellow
Cell Value is between 500 and 2000 = red

I just wanted to clarify: when I reopen the document none of the numbers have changed, but the colors switch around and e.g. the yellow gets replaced with blue. This does not happen every time I close/reopen but it has happened at least two or three times and I have no idea why.
