Disk running at 100%

I have a question about disk usage. as soon as I open my computer, the disk immediately runs at 100%, and rarely dips below 90%. It has been doing this for about a month, and I have ran chkdsk, made sure my hard drive has space, ran malware checks, and removed unused games and programs from my computer. My computer takes about 20 mins to fully boot up, and everything it does is incredibly slow. Does anyone have any ideas on what I can do to fix it?
