Disk Usage 100%

I am having hp pavillion series laptop intel i3 with windows 8 , 4gb ram and 500gb hard disk, and 2gb graphics. From last 6 months my system was working very slow after startup its was taking much time then i found that my disk usage is 100% . i tried lot of methods but only failed really i m fed up with this problem dont even wanna start my system
following are the steps i tried
-reset windows (fresh install, formatted the hard drive)
-disabled Windows Search
-disabled Superfetch
-disabled all startup apps having high impact
these things not helped me
so please provide me solution for this..so fedup with this problem

You have THIS HP Pavilion 15-n205tx Notebook PC which was purchased on June 21, 2014 and came with Windows 8.1 64-bit.
It also came with these hardware specs:
Intel Core i3-3217U 1.80 GHz dual core processor

Considering how weak its hardware is, I'm not surprised at all that it has the speed and performance of a turtle.
We have no way of knowing how well it's maintained and what's installed and running in it, so that may be part of the problem.
