FP2003 Theme Problems

I am having a problem with my site. I am using FP 2003, and my server
has frontpage extensions installed. It is through hostdime.com
My website is www.jlazy3cabinetry.com
I primarily use firefox 9 for my browser, but this is happening with
netscape and IE5 & 6
I am using the Satin theme from FP2003.

I have navigation bars at the top and bottom of the pages to get
around easier. The problem is, some of the button graphics are not
showing up. On the main page the graphic for the last button does not
show up. This happens on many of the other pages as well. When I right
click on the button and select view image (in Firefox) it goes forward
to a 404 page and says:
The requested URL /_derived/contact us.htm_cmp_Satin000_hbtn.gif was
not found on this server.

I have looked in that folder and that file is there, along with all of
the other files. I don't understand what is happening, why that button
graphic won't load. Any ideas?

Also any critique on the site would be accepted.

Hate to ask this, but it doesn't quite look like a FP issue.
Can you let me onto your FTP?

On your index page, I see that your reference to the image is Satin000_hbtn.gif (on the page that doesn't show the image), on the page that works it's satin000_hbtn.gif. Using upper case in one reference and lower case in the other.You may have just made some minor mistakes like that.
On a critique, on your products page there is no way to get back to your home page. I would think you would want a home button on that page as well.
