How Do Banner Ads Work?

This may sound like a simple question, but im actually interested more in how are they controled and placed, and how do they come to be?

So you have a website, with free features, say a forum or a page of tutorials.

members start joining and the page becomes more popular.

now what? do companies contact you asking to put ads on your site? or do you go to them?

is it an independent company, say a specific manufacturer? or is there an ad firm that acts as a middle man?

what about access? do they send you a .swf file every month that you post? or do you use a "php?include_once" tag to allow them to put whatever they want in there little cell on a table?

what about google content relevant ads on your site?

what happens if you decide you no longer want ads? or you object to the material there advertising? are there contracts?

Any insight into this process would be appreciated.

You go to them, they don't go to you (unless your site is so amazingly popular that they beg you to add their ads, but that is very very rare).

Also, just because you apply to have ads, doesn't mean you'll get them. Sometimes you'll get denied.

I know that Google uses JavaScript ads. I just place 1 small JavaScript on my site which links to their site, and they load ads that way. It's totally automatic once I add the JavaScript they provide me.

I suppose somewhere there could be an "ad firm", but all the advertisers I've come in contact with were seperate companies.
