how do i stream audio from my police scanner to a website?

I would like to stream audio from my police scanner to somewhere on the web so I can use my Smartphone to go to the website to monitor the streaming audio. How would I do this? Thanks

It would help knowing what kind of outputs you have on your scanner, like a headphone jack, etc.

The scanner has a 3.5mm jack and the computer has a 3.5mm input jack so that part is easy to hook up.
