For GHOST versions 2003 and earlier, you create a bootable floppy and boot from it to run GHOST. GHOST 2003 had a lame Windows interface that tinkered with the partition table and created a temporary boot partition to make it appear to run from Windows. The smart money is to simply run it from bootable media. I believe the GHOST 2003 CD will boot directly and allow you to run the backup as well.
I am not familiar with Ghost but he is asking how to make a bootdisk so I answered that part.
