This meets your criteria:

http://www.newegg.com/Product/Product.asp?Item=N82E16811156018

But here is what I would get for just $20 more:

or this:

It's hard to find a good power supply for under $40, much less a case with one. Normally any PS shipped with a case should be replaced.

Here's one for pretty close with a semi-decent supply.

This one's not too shabby.

Here's one, but the PS will probably die in less than a year or sooner:

10% code bensbargains = $44 + shipping.

Sorry--the cases I linked to were without a PS as I thought you said you had a good one.

Here is one ($56)--a bit over your budget, but well worth it:

http://www.newegg.com/Product/Product.asp?Item=N82E16811144026

Here is one $44.99 after rebate:

http://www.newegg.com/Product/Product.asp?Item=N82E16811208008

If you want to stay in your budget, this is probably your best choice ($29.99)--Rosewill with 400 Watt PS--can't beat that with a stick (although you'll probably be replacing the power supply in 6 months! :D ):

Yeh. I got a X-Connect PS a while back when they weren't so cheap. $40 for a 400 watt. It's an older one, of course. Still nice. Nice finishes, too. Mine's a really shiny Titanium finish. Attracts fingerprints like none other, though. Dual 80MM fans, though. Always a plus.
Only problem is, I tried putting it in one case of mine, and the MOBO cable [which is incredibly thick] wouldn't fit, due to the drivebays. The case wouldn't close completely. Also, that cable, and a couple others, are far too inflexible.
Oh, yeah. That reminds me of another perk of Ultra's PSs... UV-reactive cables!
