Microphone picks up all output audio on my pc

I have been having problems with my pc audio. When connecting mic and headphones to my computer, my mic picks up all output audio.
For example when in a call on Discord or Skype, other people can hear themselves and music/video/videogame playing on my PC.
I've searched for a solution for almost a year now. I have tried different headsets, tried to update my drivers...
If anyone knows any solutions, help is much appreciated.

Depending on your audio chipset, disable "Stereo Mix" or "What You Hear".
