Modem help please.

    Can anyone help me identify my modem? It is configured as a standard modem but I have been having a lot of disconnections lately and want to update drivers and or add some initialisation strings if I can find out the manufacturer. Windows says it is using the latest drivers.
    The modem is an external one, about 5" by 7" and about 1" high, cream in colour, no manufacturers name on it at all. On top is has printed 56000bps data/fax/voice modem and underneath it has N136 and FCC No.H52PT-3020.
    Sorry this is not much to go on but someone may recognise it.
