Online quiz Script or Form?

Anybody know where I can find a script or something that will help me develop a quiz? The quiz can probably be done a few different ways. But basically, it is intended to calculate a person's score based on the answers they chose. There are seven sections with about 10 yes or no questions in each section. If they answer yes, they get so many points. At the end of the quiz, it calculates their score and gives one of three summaries. (Imagine a Cosmo magazine quiz)

I'm imagining a check box next to each question. If the checkbox is checked, the points get calculated. But I guess I need the script to calculate it and then return the summary when they are done.

Any help would be greatly appreciated.

There are tons of free scripts in there ... and their search engine is pretty good at finding what you are looking for. Keep an eye out for contributors names ... they usually have more scripts on their websites (i.e. Kurt Grigg http://www.btinternet.com/~kurt.grigg/javascript/
