I have a lengthy form from the doctor's office that I need to fill.
I'd like to scan the document/questionnaire to my computer so that I may answer all questions via computer.
When I do the scan, it automatically wants to scan as a PDF document.
This appears to be a "Read Only" document that I can not fill in the answers.
    Question:
Is it possible to scan a document to the computer and have the freedom to answer all questions via computer?
If it is possible, how may I do it?
    Thank you for your help.
    Floyd

    Etaf,
I do have an option other than PDF however the MS Word is the one that I can edit and fill in the blanks of a questionnaire.
The problem w/Word is when scanned, the document is distorted much so from the original whereas the PDF scan is exactly like the original copy.
Also, when scanning to a MS Word doc, the blanks of a questionnaire are nearly impossible to fill in w/answers.
Some blanks take filling in, some don't.
This is why I'd like to convert PDF to either Word or Excel.
I do see some software available to be purchased but I'd rather do it for free if possible.

Thank you for your helpful reply,
Originally, I could not get my questionnaire to download to the site but it is doing it now.
This process appears to be getting done online.
This is my first time at attempting to accomplish this so I'm wondering,
Would such pertinent data as Social Security numbers, etc. be taken by the website "fillanypdf"?
    Thanks again for your help

    Thank you etaf,
I had attempted this earlier but I must have done something wrong since the questionnaire would not download to the website. It is doing so now and I have replied to the earlier post of md2lgyk as well.
I have asked md2lgyk if any pertinent data such as Social Security numbers, etc. could be basically hijacked since this appears to be converted online.
I will wait to mark "solved" when I learn about the SS #s ,etc.
Thanks again for your help and have a great day.
