Could be a corrupted icon cache. Delete the cached files in C:\Users\name\AppData\Local\Microsoft\Windows\Explorer
and reboot or logout and log in again.

There is another IconCache.db in C:\Users\name\AppData\Local which you can remove.

For permissions you need to set user rights at the top level (C:\Users\<name>) and give the user Full Control and as long as the folders below have inheritance enabled for sub folders, they should all get the new rights filtered down.

The picture on your icons indicates that that particular folder or file is shared. Have you shared your files recently? To check, right click on the file/folder, click on properties, then click on the "sharing" tab.

    Good luck!
